Submit a Prayer Request

Click the button below to submit a prayer request to the Faith Church family. Submissions to this form are shared internally with all Faith Church attenders and members who are subscribed to the list, and may be included in printouts of the weekly prayer list which is available on Sundays. Your first and last name along with your prayer request will be shared. If you don't have an email address, you can also call the church office at 262-728-6496 to submit your request over the phone.
